The Daughters of Charity Services has the following positions available: Director, Community Care
Develops and maintains collaborative relationship with appropriate service providers, public benefit and social service provider agencies as appropriate.Work closely with senior management staff on clinical excellence in community based programs. To include mobile health units, programs for homeless patients, and community based nursing and behavioral health programs. Assist with grant research, writing, implementation and reporting. Must have at least a bachelor's degree. MPH strongly preferred. Bilingual/bicultural strongly preferred. (Proficient in Spanish language). Five (5) years of progressive experience preferred.
Case Manager
The case manager's role is to facilitate the patients' use of the health care system to achieve the best possible results for the patient. This requires referrals and follow-up thorough enough to integrate and procure the services needed by patients who are homeless, migrants or otherwise lack access in various sites. The case manager's work is crucial to empowering patients access to continuum of care and to achieve optimal health and level of functioning. Bachelor's degree or higher in social work or related human services field preferred. At least three (3) years of case management experience preferred. Bilingual/bicultural strongly preferred. Works closely with senior management and staff on clinical excellence in community based programs. Program Director, Registered Nurse Mobile Health
The Program Director is an RN with advanced training in the maternal-newborn clinical specialty who practices, teaches, consults, supervises, coordinates and oversees the prenatal/postpartum/ neonatal services for the mobile unit. Valid Louisiana RN and Advanced Practice Registered Nurse (Clinical Nurse Specialist) licenses. BSN andMasters Degree in nursing/public health required. Trained in Centering Pregnancy® model of care. Certification in BLS for Health Care Providers. Oral and written communication, analysis and problem solving skills. Bilingual/bicultural required. (Proficient in Spanish language). Documented leadership exp. and work within healthcare arena.
Certified Medical Assistant
Must possess a sense of discipline to work in accordance with accepted nursing and medical standards. Displaying excellent reading, and verbal and nonverbal communication skills. Responsible for greeting the public, scheduling appointments, maintaining electronic medical records and clerical duties as assigned. Process patients at admission and discharge. Must have a high school diploma or equivalent with at least three (3) years exp. in a health care business office setting. Must have excellent communication skills, including verbal and written.
